STEP 2: CONFERENCE

The second step in PACE Care's Care Planning process is the Conference. Think of the Conference like a family huddle. One of our Senior Care Managers will come to your home, or wherever your loved one lives, and facilitate a discussion with your family about your needs and preferences when it comes to long-term care. 


Our Senior Care Managers will also perform a comprehensive home safety review. With your permission, he/she will check the home for potential problem areas.  The home safety review is intended to reduce the risk of injury to your loved one at home.

WHAT HAPPENS DURING THE CONFERENCE?

During the conference, our Senior Care Manager will perform the following assessments and reviews:
IN-HOME ASSESSMENT
  • Needs and Preferences Analysis
  • Assessment of Independence in Activities of Daily Living (ADLs)
  • Daily Routines
  • Medication Regime/Routines
  • ​Interests and Activities
COMPREHENSIVE HOME SAFETY REVIEW
  • ​Electrical/Fire Safety Equipment
  • Fall Prevention Tips
  • Placement of appliances and furniture
  • Rugs, cords and other common tripping hazards
  • Bathroom/shower Safety Review
  • ​Proper labeling and storage of medicine and other medical equipment

STEP 3: PLAN FOR CARE

When it comes to long-term care, there are many options to choose from; each with its own distinct advantages and disadvantages. We want you to know your options so that you can make the right decision. PACE Care has Gerontologists on staff who help families plan for the long-term care of their elderly relatives.

WHAT IS A CARE PLAN?

A care plan is the product of the collaboration that happens during the conference. Your family and the Senior Care Manager create a  "road map" for long-term care that takes into account the family's current and future needs.   It facilitates the flow of care such as personal care, food and nutrition, and light housework in the manner which the client prefers. 

​Further it can help illuminate support areas that may need intervention for example, fall prevention, home modification, and need for assistive devices and technology.
 
Care Plan Main Goals
  1. Ensures the needs of the client/patient are communicated and followed by caregiver on duty 
  2. Establishes a living and evolving communication document that helps maintain consistency and quality of care for your loved one.
  3. Provides health care professionals a record of changes in routine, eating habits, cognitive and physical abilities as well as emotional mood.
